Only the very pricey insurance packages cover routine veterinary care like vaccinations, neutering, flea and worm treatments etc, and only a select few companies offer them. Since future insurance premiums rise in accordance with the number of times the insurance is claimed on, they're rarely viable in the long run. I'd say $20 a month is mighty cheap for a plan which supposedly includes all routine healthcare and has reasonable excess charges to boot.

I'm not aware of an insurance company which allows you to take out less than a years' insurance; the idea of pet insurance is to have a back-up if the worst happens, rather than to take chances - insurance companies wouldn't make any money if they allowed their customers to take out insurance only to cover select times when their pet actually needed treatment.

I don't think pet insurance covers spaying or neuturing... I think it's just for sicknesses and accidents.

I'm not 100% sure about the US, but pet insurance doesn't usually cover the normal standard non-emergency things like vaccinations and neutering.
